San Francisco Health Plan Offers Affordable, Comprehensive Health Care Coverage


They have four unique health care coverage programs that provide complete medical, dental, and vision care at a very low-cost.    

Eligibility in the programs depends on annual income, family size, and in some cases, an applicant's age.  Select from the program menu above for more information on benefits, services, eligibility, and more.

Free and Low-Cost Health Insurance Options

San Francisco Healthy Plan for Children and  Young Adults, or known also as San Francisco Health Plan, is a health insurance program offered by San Francisco Health Plan (SFHP), providing affordable health coverage to low- and middle-income residents living in San Francisco. Approximately half of San Francisco County's 10,000 uninsured children qualify for this program. Delta Dental contracts with the SFHP to provide dental services to children and young adults of low-income families in San Francisco. This site is in English, Spanish, and Chinese.

 San Francisco Department of Public Health offers a full listing of insurance options for families. Options include COBRA, private sector, public sector options, such as Medi-Cal, Healthy Families, Healthy Kids, Children’s Medical services, and Free and Low-Cost Health Care Services in San Francisco.

• Healthy San Francisco is San Franciso's innovative health access program.  As of February 10, 2009, San Francisco residents with an income at or below 500% of the Federal Poverty Level (for one person $54,150; for a family of four $110,250) are eligible to enroll in this one-of-a-kind access program.  Click here to see if you may qualify. If eligible, you may join Healthy San Francisco regardless of immigration status or pre-existing medical conditions.

The Community Health Network
The San Francisco CHN encompasses a wide array of services across a continuum of care.  You do not have to have insurance for some of these services. Major service components include primary care (provided at sites throughout the City), specialty care, acute care, home care, long-term care, and emergency care.  If you need a TB shot or child's physical, visit one of the Health Centers for free or low cost care.
